After verbally reporting your claim, you must immediately file a written claim report, including any third-party claims.
Fill out the auto insurance claim form, complete the supporting documents and provide a full chronology of the incident that caused the loss.
If the claim is submitted through a CPR auto workshop, the survey will be carried out by a CPR auto workshop survey officer. Meanwhile, when submitting a claim that does not go through a CPR auto workshop, the survey will be carried out by a survey officer from Chubb who will contact you to determine the time and location of the survey.
You must not modify or replace any damaged properties belonging to you or the third-party involved in the incident without prior written consent from Chubb.
If the person submitting the claim is not the insured or the name stated in the policy, a power of attorney letter signed by the insured must be submitted.
Provide accurate and truthful information about the loss.
If a vehicle is stolen or lost, a police report stating the loss must be submitted as a supporting document.
*Coverage shall apply in accordance with the Indonesian Motor Vehicle Insurance Police you hold.
Supporting Documents for Partial Loss Claim
A fully completed claim form;
Copy of your auto insurance policy;
Copy of driver’s driving license and ID card of the insured, according to the name stated in the vehicle’s registration certificate (Surat Tanda Nomor Kendaraan);
Copy of the vehicle’s registration certificate (Surat Tanda Nomor Kendaraan);
A police report if the loss resulted from criminal act committed by another person; and
If the incident causes any third-party loss, the following documents must be submitted:
A statement letter from the third-party informing their demand of a compensation;
A statement letter from the third-party informing that their car is not insured;
A motor vehicle accident police report;
Copy of the third-party’s driving license and vehicle’s registration certificate (Surat Tanda Nomor Kendaraan);
Photos of the damaged third-party vehicle; and
Other documents relevant with the third-party.
Supporting Documents for Total-Loss Claim
Original auto insurance policy;
Original vehicle ownership book (Buku Pemilik Kendaraan Bermotor), vehicle’s registration certificate (Surat Tanda Nomor Kendaraan), and vehicle’s invoice;
Original & spare ignition keys;
Copy of the insured’s ID card, according to the name stated in the vehicle’s registration certificate (Surat Tanda Nomor Kendaraan);
Copy of driver’s driving license;
2 (two) duplicates of blank receipt and 1 (one) duplicate with stamp duty;
Original police report;
Documentations required in accordance with applicable regulations of motor vehicles belonging to diplomatic or international agencies;
Original vehicle inspection book (KIR) for vehicles subject to mandatory KIR;
Confirmation letter from the Regional Police office (Kaditserse/ Diskrimum);
Letter from the Sistem Administrasi Manunggal Satu Atap (Samsat) about blockage of vehicle’s registration certificate (Surat Tanda Nomor Kendaraan); and
Subrogation/property transfer letter.
Supporting Documents for Loss of More Than 75% of Vehicle’s Actual Price (Constructive Total Loss)
Original auto insurance policy;
All supporting documents required for a Total-Loss claim, except for document number 10, 11, and 12; and
Letter of loss amount estimation from one of our partnering workshops.
A Police Report is Required for the Following Claims
Claim of loss due to theft, both vehicle or standard/non-standard vehicle accessories.
When involving a third-party.
Loss/damage due to other people’s criminal act (malicious damage).
If the vehicle sustained total damage due to an accident.
