How to make a Financial Lines claim

How to report a claim
Please include the following information with any claim notification:
- The contact details of the person in your company who will be dealing with the claim.
- The insured person's name
- Your policy number
- The date of loss
- A brief summary of facts and circumstances
- Please refer to the individual products below for any additional information that may be required.
- Email your information to our claims team.
What to submit
Directors' & Officers' liability claim
- Copies of any relevant correspondence including letters of demand
- Copies of pleadings
- Supporting documentation for the Insured Person’s position
Employment Practices liability claim
- Copies of any relevant correspondence including letters of demand
- A copy of the complainant’s employment contract
- A copy of any pleadings filed including Fair Work Applications
Cyber claim
- Nature of cyber incident including a brief summary of facts and circumstances
- Confirmation as to whether an IT specialist been instructed. If so, please provide details of the IT specialist and copies of any retainer.
- A copy of any IT reports or screenshots of the cyber incident
Crime claim
- Anticipated quantum
- Details as to whether any forensic accountants have been appointed; if so, please provide details and a copy of any retainer
- Details of whether the police have been notified; if so, please provide the incident number and contact details for the police investigator
Professional Indemnity claim
- A copy of any agreements or contracts in dispute
- Copies of any relevant correspondence including letters of demand
- Copies of pleadings
General claim
- Copies of any relevant correspondence
- Copies of pleadings (if relevant)
