- Policyholder's Name/Insured
- Policy number
- Type of policy
- Brief summary of facts and circumstances
- Full contact details of the person within your company to whom enquiries relating to the claim should be directed
- Date of loss
- Name of claimant
- Copies of any incident report
- Copies of any relevant correspondence
- Copies of pleadings (if relevant)
