Non-Contractual Changes

1. Change of address or contact information
Required Documents:

  • The Policy Change and Reinstatements Request Form
  • A certified true copy of your House Registration Certificate or Identity Card (if any)


2.Changes of name/family name of the Insured
Required Documents:

  • The Policy Change and Reinstatements Request Form
  • A certified true copy of the deed poll(name-change certificate)


3.Change of beneficiary
Required Documents:

  • The Policy Change and Reinstatements Request Form
  • A certified true copy of the Identity Card and House Registration Certificate of the new proposed beneficiary


4.Change of misstatement of age/gender
Required Documents:

  • The Policy Change and Reinstatements Request Form
  • A certified true copy of the Birth Registration or Identity Card or House Registration Certificate
  • Policy Document and Processing fee of 200 baht.
  • Additional premium (if any) since the change in age and sex may result in a change to the insurance premium amount


5.Change in mode of premium
Required Documents:

  • The Policy Change and Reinstatements Request Form
  • New mode premium


6.Requesting a new policy document due to loss of the original document
Should the policy document be lost, destroyed, or damaged entirely, the policy owner must notify the company and request a new policy document replacement.

Required Documents:

  • The Policy Change and Reinstatements Request Form specifying cause of such loss or damage
  • Police Report
  • A certified true copy of your Identity Card
  • Processing fee of 200 baht per policy document


7.Requesting a new policy-owner card due to loss of the existing one
Required Documents:

  • The Policy Change and Reinstatements Request Form
  • Request Form specifying cause of such loss
  • A certified true copy of your Identity Card
  • Processing fee of 30 baht per card

After the company receives the completed documents, the Insured will receive a new duplicate policy document or a new policy-owner card.


8.Change of signature
A change of signature is considered effective upon receipt of all supporting documents.

  • Sign your previous and new signature on the Policy Change and Reinstatements Request Form
  • Provide a certified true copy of your Identity Card with both the previous and new signatures

Note: After the non-contractual change or changes have been recorded, the Insured will receive a policy endorsement identifying the details of the change(s).
