Accident & Health

Hospital Income/Daily Benefit Claims (DMs; HCP & GHIX)

  • Duly completed claim form (Accident & Sickness Proof of Loss Form)
  • Detailed statement of account
  • CTC (Certified True Copy) of admitting history (optional, but preferred)
  • CTC of discharge summary
  • Official receipts for surgeon’s fee (where there is surgical benefit cover under the policy and a surgical procedure has been performed)

Medical Expense Reimbursement Claim (PA, HE, MM & GAHI)

  • Duly completed claim form
  • Original copy of medical bills (including itemized charge slips and professional fees) and receipts
  • Official reports pertaining to the accident (e.g.,police report, accident report, etc.), if any
  • Emergency room records
  • All medical records pertaining to the accident
  • Prescriptions for medicines

Disability Claim (PA)

  • Duly completed claim form
  • CTC of admitting history
  • Supplementary Medical Report (SMR) indicating physician’s prognosis and time of disability

Dismemberment Claim (PA)

  • Duly completed claim form
  • Official accident report (e.g. police report, inter-office accident report, newspaper clippings, etc.)
  • CTC of admitting history and operating room records (if any)

Death Claim (PA)

  • Duly completed claim form
  • Duly registered death certificate
  • Duly registered birth certificate of insured
  • Autopsy report/medico-legal statement
  • Official police report and other related reports (e.g.,inter-office accident report, newspaper clippings, etc.)
  • Affidavit/s from witness/es
  • Photos taken at incident scene (if available)
  • Proof of claimant’s relationship to beneficiary (e.g.,marriage certificate, birth certificate, baptismal certificate,etc.)

Travel Inconvenience

  • Duly completed travel insurance claim form
  • Letter from card member describing situation and related expenses (all claims)
  • Property irregularity report (luggage loss/delay)
  • Letter from airline confirming delay (flight delay/missed flight)
  • Record of costs incurred in purchase of airline ticket
  • Statement of account in which charge for airline ticket appears, and/or the airline ticket itself
  • Record of charges and receipts for expenses incurred (all claims)
