is domiciled. After you complete the notification form, it is electronically sent to the appropriate multinational claim
professional for attention.

When phoning or faxing claims, please provide as much detail as is available. This should include, but not be limited to:
- Insured's Name
- Contact Name
- Chubb Policy #
- Date of loss
- Location of loss
- Cause of loss
- Your Policy or Reference #
- Initial steps taken to mitigate the loss
- Estimated amount of loss
